What are Psychosocial Rehabilitation Services?
Psychosocial rehabilitation services provide an opportunity for consumers to:

  • Increase their function in their environment of choice with the least amount of ongoing professional intervention
  • Improve management of the his/her illness and facilitate recovery
  • Increase and maintain competence in normal life activities

The basic goals of psychosocial rehabilitation services are:

  • Development of individual’s skills
  • Education on self-management of symptoms, medications and side effects

Who is eligible for Psychosocial rehabilitation services?

  • Individuals with primary mental health issues
  • Individuals who are 18 years or older

What do Psychosocial rehabilitation services enhance?

  • Individual or group skill building 
  • Problem solving techniques
  • Medication management
  • Cognitive and psychosocial functioning
  • Illness management

Psychosocial rehabilitation services provides professional services such as:

  • Adult day services up to 5 hours/5 days per week.
  • Individual, group and family skill training.
  • Psychiatric services.
  • Pharmacy services.
  • Nursing services.
  • Medication administration.
  • Consultation and education.
  • Community Support Individual (CSI).
  • Community Support Team (CST).
  • Case coordination services.
  • Crisis and emergency services.

Fees:
According to income (sliding fee scale)
We accept private insurance, Medicare, and Medicaid
